    Effect of speaker age on speech recognition and perceived listening effort in older adults with hearing loss

    Published online January 2012Purpose: Older adults exhibit difficulty understanding speech that has been experimentally degraded. Age-related changes to the speech mechanism lead to natural degradations in signal quality. We tested the hypothesis that older adults with hearing loss would exhibit declines in speech recognition when listening to the speech of older adults, compared with the speech of younger adults, and would report greater amounts of listening effort in this task. Methods: Nineteen individuals with age-related hearing loss completed speech recognition and listening effort scaling tasks. Both were conducted in quiet, when listening to high and low predictability phrases produced by younger and older speakers respectively. Results: No significant difference in speech recognition existed when stimuli were derived from younger or older speakers. However, perceived effort was significantly higher when listening to speech from older adults, as compared to younger adults. Conclusions: For older individuals with hearing loss, natural degradations in signal quality may require greater listening effort. However, they do not interfere with speech recognition – at least in quiet. Follow-up investigation of the effect of speaker age on speech recognition and listening effort under more challenging noise conditions appears warranted

    Constraining spatial pattern of early activity of comet 67P/C-G with 3D modeling of the MIRO observations

    Our aim is to investigate early activity (July 2014) of 67P/CG with 3D coma and radiative transfer modeling of MIRO measurements, accounting for nucleus shape, illumination, and orientation of the comet. We investigate MIRO line shape information for spatial distribution of water activity on the nucleus during the onset of activity. During this period we show that MIRO line shape have enough information to clearly isolate contribution from Hapi and Inhotep independently, and compare it to the nominal case of activity from the entire illuminated surface. We also demonstrate that spectral line shapes differ from the 1D model for different viewing geometries and coma conditions relevant to this study. Specifically, line shapes are somewhat sensitive to the location of the terminator in the coma. At last, fitting the MIRO observations we show that the Imhotep region (possible extended source of H2_{2}O due to CO2_{2} activity) contributes only a small fraction of the total number of water molecules into MIRO beam in the early activity. On the other hand, a strong enhancement of water activity from the Hapi region seems required to fit the MIRO line shapes. This is consistent with earlier Rosetta results. Nevertheless, within the assumption of our coma and surface boundary conditions, we cannot get a reasonable fit to all MIRO mapping observations in July 2014, which may illustrate that a more sophisticated coma model or more accurate temperature/velocity distribution is needed.Comment: 22 pages, 15 figures, submitte

    Health-related quality of life in women with endometriosis:psychometric validation of the Endometriosis Health Profile 30 questionnaire using confirmatory factor analysis

    STUDY QUESTION: Which of the competing models of the Endometriosis Health Profile 30 Questionnaire (EHP-30) factor structure is best supported by confirmatory factor analysis (CFA)? SUMMARY ANSWER: Findings support a five-factor first-order model of the EHP-30, thereby lending support to the model originally suggested by the questionnaire developers. WHAT IS KNOWN ALREADY: Endometriosis has a negative impact on quality of life, and measures specifically developed to address this impact, such as the EHP-30, are vital in research and disease management. Previous studies have found different models of the EHP-30 factor structure, and generated uncertainty regarding how to use the questionnaire. CFA can be applied to compare competing factor models and determine the underlying structure of a questionnaire. STUDY DESIGN, SIZE, DURATION: This cross-sectional multicenter study included 304 women with endometriosis recruited from three different public health service endometriosis clinics (referral centers for treatment of severe endometriosis) and the Danish Endometriosis Patients Association from 2014 to 2015. PARTICIPANTS/MATERIALS, SETTING, METHODS: Diagnosis of endometriosis was confirmed in medical records for 84.2% and by histology for 66.8% of participants. Questionnaires (the licensed Danish version of the EHP-30) were sent by post two times with a 6- to 12-week interval. CFA was used to examine construct validity and Bland–Altman plots to examine test–retest reliability and the convergent validity with the Short Form 36 version 2. MAIN RESULTS AND THE ROLE OF CHANCE: Response rate was high (87.6%). CFA supported the original first-order five-factor structure of the EHP-30, and thereby, the use of five separate scale-scores in clinical and research practice. Visual inspection of Bland–Altman plots suggested excellent test–retest reliability of the EHP-30 and supported the use of a disease specific quality of life instrument for women with endometriosis. LIMITATIONS, REASONS FOR CAUTION: Diagnosis could not be confirmed through histology data in 33.2% of participants. However, subgroup analyses based on women with confirmed histology only, yielded similar results. Data related to menstrual cycle stage and the use of hormonal and pain medication during questionnaire completion were not collected. A larger study, including data from different countries on different continents, would be better designed to exclude potential population bias. WIDER IMPLICATIONS OF THE FINDINGS: EHP-30, with its original five-factor structure, appears to be a valid, stable, and specific quality of life measure for women with endometriosis. It seems easy to understand, quick to administer, and importantly, scoring might be unaffected by cyclical/menstrual pain symptoms related to endometriosis. The finding of a five-factor model from different studies across several countries supports the crosscultural validity of the EHP-30.     Prognostic and Mechanistic Potential of Progesterone Sulfates in Intrahepatic Cholestasis of Pregnancy and Pruritus Gravidarum

    A challenge in obstetrics is to distinguish pathological symptoms from those associated with normal changes of pregnancy, typified by the need to differentiate whether gestational pruritus of the skin is an early symptom of intrahepatic cholestasis of pregnancy (ICP) or due to benign pruritus gravidarum. ICP is characterized by raised serum bile acids and complicated by spontaneous preterm labor and stillbirth. A biomarker for ICP would be invaluable for early diagnosis and treatment and to enable its differentiation from other maternal diseases. Three progesterone sulfate compounds, whose concentrations have not previously been studied, were newly synthesized and assayed in the serum of three groups of ICP patients and found to be significantly higher in ICP at 9-15 weeks of gestation and prior to symptom onset (group 1 cases/samples: ICP n = 35/80, uncomplicated pregnancy = 29/100), demonstrating that all three progesterone sulfates are prognostic for ICP. Concentrations of progesterone sulfates were associated with itch severity and, in combination with autotaxin, distinguished pregnant women with itch that would subsequently develop ICP from pruritus gravidarum (group 2: ICP n = 41, pruritus gravidarum n = 14). In a third group of first-trimester samples all progesterone sulfates were significantly elevated in serum from low-risk asymptomatic women who subsequently developed ICP (ICP/uncomplicated pregnancy n = 54/51). Finally, we show mechanistically that progesterone sulfates mediate itch by evoking a Tgr5-dependent scratch response in mice. Conclusion: Our discovery that sulfated progesterone metabolites are a prognostic indicator for ICP will help predict onset of ICP and distinguish it from benign pruritus gravidarum, enabling targeted obstetric care to a high-risk population.     Regional unit definition for the nucleus of comet 67P/Churyumov-Gerasimenko on the SHAP7 model

    The previously defined regions on the nucleus of comet 67P/Churyumov-Gerasimenko have been mapped back onto the 3D SHAP7 model of the nucleus (Preusker et al., 2017). The resulting regional definition is therefore self-consistent with boundaries that are well defined in 3 dimensions. The facets belonging to each region are provided as supplementary material. The shape model has then been used to assess inhomogeneity of nucleus surface morphology within individual regions. Several regions show diverse morphology. We propose sub-division of these regions into clearly identifiable units (sub-regions) and a comprehensive table is provided. The surface areas of each sub-region have been computed and statistics based on grouping of unit types are provided. The roughness of each region is also provided in a quantitative manner using a technique derived from computer graphics applications. The quantitative method supports the sub-region definition by showing that differences between sub-regions can be numerically justified

    Naupliar and Metanaupliar development of Thysanoessa raschii (Malacostraca, Euphausiacea) from Godthåbsfjord, Greenland, with a reinstatement of the ancestral status of the free-living Nauplius in Malacostracan evolution

    The presence of a characteristic crustacean larval type, the nauplius, in many crustacean taxa has often been considered one of the few uniting characters of the Crustacea. Within Malacostraca, the largest crustacean group, nauplii are only present in two taxa, Euphauciacea (krill) and Decapoda Dendrobranchiata. The presence of nauplii in these two taxa has traditionally been considered a retained primitive characteristic, but free-living nauplii have also been suggested to have reappeared a couple of times from direct developing ancestors during malacostracan evolution. Based on a re-study of Thysanoessa raschii (Euphausiacea) using preserved material collected in Greenland, we readdress this important controversy in crustacean evolution, and, in the process, redescribe the naupliar and metanaupliar development of T. raschii. In contrast to most previous studies of euphausiid development, we recognize three (not two) naupliar (= ortho-naupliar) stages (N1-N3) followed by a metanauplius (MN). While there are many morphological changes between nauplius 1 and 2 (e.g., appearance of long caudal setae), the changes between nauplius 2 and 3 are few but distinct. They involve the size of some caudal spines (largest in N3) and the setation of the antennal endopod (an extra seta in N3). A wider comparison between free-living nauplii of both Malacostraca and non-Malacostraca revealed similarities between nauplii in many taxa both at the general level (e.g., the gradual development and number of appendages) and at the more detailed level (e.g., unclear segmentation of naupliar appendages, caudal setation, presence of frontal filaments). We recognize these similarities as homologies and therefore suggest that free-living nauplii were part of the ancestral malacostracan type of development. The derived morphology (e.g., lack of feeding structures, no fully formed gut, high content of yolk) of both euphausiid and dendrobranchiate nauplii is evidently related to their non-feeding (lecithotrophic) status

    On deviations from free-radial outflow in the inner coma of comet 67P/Churyumov–Gerasimenko

    The Optical, Spectroscopic, and Infrared Remote Imaging System (OSIRIS) onboard the European Space Agency's Rosetta spacecraft acquired images of comet 67P/Churyumov–Gerasimenko (67P) and its surrounding dust coma starting from May 2014 until September 2016. In this paper we present methods and results from analysis of OSIRIS images regarding the dust outflow in the innermost coma of 67P. The aim is to determine the global dust outflow behaviour and place constraints on physical processes affecting particles in the inner coma. We study the coma region right above the nucleus surface, spanning from the nucleus centre out to a distance of about 50 km comet centric distance (approximately 25 average comet radii). We primarily adopt an approach used by Thomas and Keller (1990) to study the dust outflow. We present the effects on azimuthally-averaged values of the dust reflectance of non-radial flow and non-point-source geometry, acceleration of dust particles, sublimation of icy dust particles after ejection from the surface, dust particle fragmentation, optical depth effects and the influence of gravitationally bound particles. All of these physical processes could modify the observed distribution of light scattered by the dust coma. In the image analysis, profiles of azimuthally averaged dust brightness as a function of impact parameter b (azimuthal average, “Ā-curve”) were fitted with a simple function that best fits the shape of our profile curves (f(b;u,v,w,z)=u/bv+wb+z). The analytical fit parameters (u, v, w, z), which hold the key information about the dust outflow behaviour, were saved in a comprehensive database. Through statistical analysis of these information, we show that the spatial distribution of dust follows free-radial outflow behaviour (i.e. force-free radial outflow with constant velocity) beyond distances larger than ∼11.9 km from the comet centre, which corresponds to a relative distance of about 6 average comet radii from the comet centre. Hence, we conclude that beyond this distance, and on average, fragmentation and gravitationally bound particles are negligible processes in determining the optically scattered light distribution in the innermost coma. Closer to the nucleus we observe dust outflow behaviour that deviates from free-radial outflow. A comparison of our result profiles with numerical models using a Direct Simulation Monte Carlo (DSMC) approach with dust particle distributions calculated using a test particle approach has been used to demonstrate the influence of a complex shape and particle acceleration on the azimuthal average profiles. We demonstrate that, while other effects such as fragmentation or sublimation of dust particles cannot be ruled out, acceleration of the dust particles and effects arising from the shape of the irregular nucleus (non-point source geometry) are sufficient to explain the observed dust outflow behaviour from image data analysis. As a by-product of this work, we have calculated “Afρ” values for the 1/r regime. We found a peak in the coma activity in terms of Afρ (normalised to a phase angle of 90°) of ∼210 cm 20 days after perihelion. Furthermore, based on simplified models of particle motion within bound orbits, it is shown that limits on the total cross-sectional area of bound particles might be derived through further analysis.     European guideline on IgG4-related digestive disease – UEG and SGF evidence-based recommendations

    The overall objective of these guidelines is to provide evidence-based recommendations for the diagnosis and management of immunoglobulin G4 (IgG4)-related digestive disease in adults and children. IgG4-related digestive disease can be diagnosed only with a comprehensive work-up that includes histology, organ morphology at imaging, serology, search for other organ involvement, and response to glucocorticoid treatment. Indications for treatment are symptomatic patients with obstructive jaundice, abdominal pain, posterior pancreatic pain, and involvement of extra-pancreatic digestive organs, including IgG4-related cholangitis. Treatment with glucocorticoids should be weight-based and initiated at a dose of 0.6–0.8 mg/kg body weight/day orally (typical starting dose 30-40 mg/day prednisone equivalent) for 1 month to induce remission and then be tapered within two additional months. Response to initial treatment should be assessed at week 2–4 with clinical, biochemical and morphological markers. Maintenance treatment with glucocorticoids should be considered in multi-organ disease or history of relapse. If there is no change in disease activity and burden within 3 months, the diagnosis should be reconsidered. If the disease relapsed during the 3 months of treatment, immunosuppressive drugs should be added

    Cilj je rada skrenuti pozornost na nekoliko elemenata bitnih za zavičajne zbirke i ukazati na njihov značaj. Prvenstveno i osnovno – lokalna je povijest temelj zavičajne zbirke. Njezino istraživanje, zasnovano na brojnim znanstveno-istraživačkim metodama, polazišna je točka za izgradnju zbirke, prezentaciju i komunikaciju sadržaja, a temeljeno na multidisciplinarnosti. Uz tradicionalne oblike prezentacije i komunikacije, knjižničarima na raspolaganju stoje i elektronički, posebice mrežna stranica. Takav pristup podrazumijeva dodatne kompetencije i afinitete zavičajnoga knjižničara. Istaknuta su dva, često zanemarena izvora za istraživanje povijesti – usmena povijest kao temelj socijalne povijesti te efemerna građa, kao „blago, a ne smeće“.The aim is to direct attention to several elements essential for the local history collections and indicate their importance. Firstly and foremost – a local history is the basis of local history collection. Research, based on a number of scientific research methods, is the starting point for building a collection, content presentation and communication, and justified on the multidisciplinary. In addition to traditional forms of presentation and communication, library materials are also available electronically, in particular web page. Such an approach entails additional competence and affinity local history librarians. Two often overlooked source for research history are featured – oral history as the foundation of social history and the ephemera, as “a treasure, not trash.
